SCHOOL HEALTH SERVICES
11TH GRADE REQUIRED PHYSICAL EXAMS
Dear Parents or Guardians:
Pennsylvania State Law requires a medical examination for ALL students in 11th (eleventh) grade including students attending PYAP, full/part time NMTCC, full/part time MCCC and PV Virtual Academy.
It is recommended that this examination be performed by your family physician since he/she
can best evaluate your child’s health, assist you in obtaining any necessary treatment, and update immunizations.
Attached is the required physical examination form (from the PA Department of Health) so you may have the examination completed prior to the beginning of school in September (ATTACHMENT BELOW).
Physical exams done after September 1, 2013 are acceptable.
If this attached form is not completed and returned to the school nurse by the first week of school,
your child will receive a physical examination by the school doctor sometime during the school year.
A copy of the PIAA sports physical is acceptable in place of the required 11th (eleventh) grade physical.
Please send a copy of the completed PIAA physical form with an updated immunization record to the school nurse.

2014/2015 IMMUNIZATION REQUIREMENTS
ALL STUDENTS in GRADES K - 12th WILL NEED THE FOLLOWING IMMUNIZATIONS:
STATE MANDATED SCREENINGS
State mandated screenings at the High School include yearly height and weight measurements, body mass index calculation and vision screening for every student. Eleventh grade students receive a hearing screening.
MEDICATION DURING SCHOOL HOURS
All medications, prescription and over-the-counter (such as Tylenol or Advil) are administered by the school nurse. Written permission is required by the student's physician and parent. Medication must be in its original container. Students are not permitted to carry their own medications (unless prior approval by the school nurse and physician for asthma inhaler and/or epinephrine auto injector).
