SCHOOL HEALTH SERVICES
2012/2013 IMMUNIZATION REQUIREMENTS
ALL STUDENTS in GRADES K - 12th WILL NEED THE FOLLOWING IMMUNIZATIONS:
  • 2 DOSES of VARICELLA (CHICKENPOX) VACCINE or HISTORY OF DISEASE
  • 4 DOSES of TETANUS (1 DOSE on or after 4th BIRTHDAY)
  • 4 DOSES of DIPTHERIA (1 DOSE on or after 4th BIRTHDAY)
  • 3 DOSES of POLIO
  • 2 DOSES of MEASLES
  • 2 DOSES of MUMPS
  • 1 DOSE of RUBELLA (GERMAN MEASLES)
  • 3 DOSES of HEPATITIS B
MEDICATION DURING SCHOOL HOURS
All medications, prescription and over-the-counter (such as Tylenol or Advil) are administered by the school nurse. Written permission is required by the student's physician and parent. Medication must be in its original container. Students are not permitted to carry their own medications (unless prior approval by the school nurse and physician for asthma inhaler and/or Epi-pen).

STATE MANDATED SCREENINGS
State mandated screenings at the High School include yearly height and weight measurements, body mass index calculation and vision screening for every student. Eleventh grade students receive a hearing screening.

11th GRADE PHYSICAL EXAMINATION
ALL Eleventh Grade students, including those attending the PYAP (Full time Vocational Techical program) or MCCC (Full time Montgomery County Community College), are required to have a physical examination completed by their family health care provider prior to the beginning of their junior year. If the school nurse does not receive a physical examination report, the school doctor will perform a physical examination.

PLEASE NOTE: The PIAA sports physical is acceptable in place of the required eleventh grade physical. Please have your child's physician attach an updated immunization record to the sports physical.
